Care Dementia Tracker is a collaborative coordination, logging, and routine tracking application designed purely for educational and organizational purposes. This application is NOT a medical device and does NOT provide medical advice, diagnosis, or treatment.
All charts, timelines, logs, and exported PDF reports are structured daily observations to assist families and caregivers in coordination. They do not replace professional medical evaluations, clinical consultations, or direct treatment instructions from licensed medical professionals (such as your loved one's neurologist, primary care physician, or psychiatrist). Never ignore, override, or delay seeking professional medical attention, and never alter any medication dosages or care plans based on data entered in or generated by this application.

Care Operations Playbook
To use Care Dementia Tracker effectively, the app is organized into five main sections. Below is the complete step-by-step caregiver playbook for managing care operations.
1. Dashboard Screen (Logging Moods, Vitals, & Quick Logging)
The central control deck. Used for real-time check-ins, tracking daily baselines, logging current emotions, and quick behavior stamps.
- Open Mood Selector: Tap the
Current Moodcircle card on the Dashboard. - Select Mood: Choose the matching emotional state from the popup grid:
- Happy / Positive (Registers as 100% score)
- Calm / Neutral (Registers as 80% score)
- Tired (Registers as 60% score)
- Sad / Negative (Registers as 40% score)
- Crying / Anxious / Agitated (Registers as 20% score)
- Save & Sync: Tapping the mood automatically updates the Patient Card, logs the change with the current caregiver's name, and registers a timeline event across all devices.
- Start Check-In: Tap
Log Morning Check-Inat the top of the Dashboard. - Enter Metrics:
- Sleep: Enter sleep duration (e.g., 7 hours) and sleep quality percentage (0–100%).
- Pain Level: Rate from 0 (No pain) to 10 (Severe pain).
- Vitals: Optionally input Blood Pressure (BP), Heart Rate (HR), Blood Sugar (BS), and Temperature.
- Safety Check: Mark environment safety hazards (locked drawers, path lighting secure).
- Save Baseline: Tap submit to update the dashboard bento cards, log the sleep record, and remove the check-in banner until the next morning.
2. Adding and Managing Medications (Routine Screen)
The command center for building care schedules and medication logs, featuring safety counts and refill alarms.
- Go to Routine: Tap the Routine tab in the bottom navigation bar.
- Select the
Medscategory tab and enter medication details:- Name: Enter Medication Name (e.g.,
Donepezil). - Time: Choose Start Date & Scheduled Time presets: Morning 🌅 (08:00), Noon ☀️ (12:00), Evening 🌇 (18:00), or Night 🌙 (21:00).
- Repeat: Choose Once, Daily, or Weekly (select days, e.g., Mon, Wed, Fri).
- Dose & Count: Set Dose (e.g., 1 pill), Remaining Quantity (e.g., 30), and Refill Alert threshold (e.g., 7 days/pills left).
- Refill Date: Schedule the next pharmacy refill date.
- Caregiver: Assign a caregiver or leave as "Anyone".
- Name: Enter Medication Name (e.g.,
- Tap Create Routine Task to add it to the active checklist.
On the Dashboard, checking off a medication marks it complete, registers the caregiver's name and completion timestamp, and automatically subtracts one pill from inventory. The dose is instantly locked to prevent double-dosing.
If a medication runs low (pill count ≤ 7 or refill date is within 7 days), it automatically appears in the Refills Tracker block at the bottom of the Routine screen. Tap Mark Refilled to instantly reset the pill count back to 30 and adjust the next refill date.
3. Logging Behaviors and Triggers (Behavior Tracker)
Sunsetting, wandering, or confusion incidents require shared documentation to recognize patterns.
Saving behaviors creates a cloud playbook, helping all caregivers know the exact soothing strategy.
4. Shift Handoffs (Caregiver Handoff Screen)
Transitioning between family members or hired nurses is seamless and keeps everyone aligned.
- Go to Handoff: Tap the Handoff tab in the bottom navigation bar.
- Write Shift Notes: In the text area, type a summary of the active shift (e.g., “Dad slept well but skipped his afternoon walk. Hydration was low today, please prioritize fluids tonight.”).
- Finalize & Sign Off: Select the Outgoing Caregiver and the Incoming Caregiver, then tap
Finalize Handoff.
This saves a transition record under transitionLogs and wipes the active notes field clean for the next shift caregiver.
5. Insights & Clinical Analytics
For neurologist, psychologist, or general physician appointments, open the Insights or Analytics tab. Select the desired date range (such as the past 14 days) and click Export PDF Report.
The app automatically compiles medication histories, vitals, sleep averages, and logged behavioral spikes into a professional clinical charts document that you can print, download, or email directly to your doctor's office.
Frequently Asked Questions
Can we track multiple dementia patients?
Yes. Premium subscribers can toggle between multiple patient profiles from the main dashboard. This is designed for families caring for multiple parents, or small care environments.
How do invitation codes work for caregivers?
Only the primary account holder needs a Premium subscription. Once you subscribe, you can generate team invitations. Family members and professional care staff can join your team and write/read logs completely for free.
Does the sync work offline?
Yes. Caregivers can mark items off and log behaviors while offline (e.g., in clinics or during outings). As soon as the device reconnects to a Wi-Fi or cellular network, all updates sync to the cloud database automatically.
Is the data secure and private?
We prioritize care data privacy. All logged behaviors, medication lists, and shift handoffs are fully encrypted in transit and at rest. We never sell or share patient health records with medical insurance providers or ad networks.
How do I manage my premium plan?
Your billing is managed entirely by Google Play. You can upgrade, change payment methods, or cancel subscription auto-renewals at any time through Google Play Store subscription settings.
