Please read these terms carefully before using Care Dementia Tracker.
Acceptance of Terms
By downloading, installing, or using the Care Dementia Tracker mobile app (“App” or “Service”), you agree to be bound by these Terms of Service (“Terms”). If you do not agree, do not use the App.
These Terms apply to all users, including caregivers, family members, and professional care team members.
Description of the Service
Care Dementia Tracker is a productivity and coordination tool designed to help families and professional caregivers organize daily care for people living with dementia. It helps you:
- check_circle Assign and track routines, medications, meals, and activities
- check_circle Log behaviors and interventions
- check_circle Create seamless digital handoffs between caregivers
- check_circle Maintain a shared health dashboard and generate care reports
The App is intended solely to help caregivers keep patients on task with their daily routines and improve family coordination. It is not a medical device and does not replace professional care.
Your Account & Responsibilities
You must be at least 18 years old and have the legal authority to manage care for the patient. You are responsible for:
- The accuracy of all information you enter about the patient
- Ensuring task reminders are acted upon correctly
- Inviting only trusted family members and caregivers
- Keeping your login credentials secure
- Complying with all applicable laws regarding patient care and data privacy
Subscriptions & Payments
The core app is free to download. Premium features (unlimited family access, PDF exports, advanced reporting) require a paid subscription. One premium subscription unlocks the full app for your entire care team.
Payments are processed securely through the Apple App Store or Google Play. Subscriptions automatically renew unless canceled at least 24 hours before the end of the current period.
Your Content & License
You own the data you enter. By using the App, you grant us a limited license to host, display, and process that data solely to provide the Service to you and your invited care team.
Prohibited Uses
You may not use the App to:
- Enter false or misleading information
- Share patient data with unauthorized parties
- Use the App for any illegal purpose or in violation of any healthcare regulations
- Attempt to reverse-engineer or harm the App
Critical Disclaimers & Limitation of Liability

The App is an Organizational Tool Only
Care Dementia Tracker exists solely to help caregivers keep dementia patients on task with daily routines, reminders, behavior tracking, and care coordination. It is not a medical device, monitoring system, or substitute for professional medical care, nursing supervision, or physician oversight.
NO WARRANTIES
The Service is provided “AS IS” and “AS AVAILABLE” with no warranties of any kind, express or implied, including but not limited to accuracy, reliability, availability, or fitness for any particular purpose.
FULL RELEASE OF LIABILITY — YOU AGREE THAT:
Care Dementia Tracker, its owners, employees, affiliates, and partners are completely absolved and released from any and all liability for any and all claims, damages, losses, injuries, illness, death, emotional distress, or expenses arising out of or related to:
- The use, misuse, or inability to use the App
- Any task reminder, behavior log, handoff note, or report generated by the App
- Any action taken (or not taken) by you or any caregiver based on information in the App
- The health, safety, well-being, or medical outcome of the patient
- Any decision made by family members, professional caregivers, or medical providers
- Any error, delay, or failure in notifications or data syncing
You voluntarily assume all risks associated with dementia caregiving. All care decisions remain entirely your responsibility and the responsibility of qualified medical professionals. This App does not guarantee patient safety or compliance with any care plan.
Indemnification
You agree to defend, indemnify, and hold harmless Care Dementia Tracker from any claims, damages, or expenses arising from your use of the App or violation of these Terms.
Termination
We may suspend or terminate your access at any time if you violate these Terms. You may cancel your subscription at any time through the App Store or Google Play.
Governing Law
These Terms are governed by the laws of the United States and the State of Arkansas, without regard to conflict of law principles.
Changes to These Terms
We may update these Terms from time to time. Continued use of the App after changes are posted constitutes your acceptance of the new Terms.
